Why Do People Who Exercise Regularly Still Suffer Sudden Cardiac Death?

At 9:50 p.m. on March 24, 2026, the passing of Mr. Zhang Xuefeng, a renowned postgraduate entrance examination instructor, due to sudden cardiac death was confirmed. At just 41 years old, he was in the prime of life. The shocking and heartbreaking news has once again brought the topic of sudden cardiac death to public attention.

Sudden cardiac death refers to natural, sudden, and unexpected death caused by cardiac conditions such as malignant arrhythmia and acute myocardial infarction, usually occurring within one hour after symptom onset. According to the Chinese Epidemiological Survey of Sudden Cardiac Death, approximately 550,000 people die from sudden cardiac death in China every year. The proportion of victims aged 18–35 surged from 12% in 2015 to 28% in 2024.

Young and middleaged people with longterm smoking, staying up late, and high mental stress, as well as patients with hypertension, diabetes, and hyperlipidemia, are all highrisk groups for sudden cardiac death. In addition to controlling “three highs,” quitting smoking, limiting alcohol, and maintaining regular routines, regular physical examinations can detect problems early for timely intervention. Electrocardiogram, echocardiography, and Holter monitor are basic tools for cardiac screening.

Director Li Yanlin, a specially invited consultant in cardiology at Shanghai Donglei Brain Hospital, noted from public information that Zhang Xuefeng exercised regularly. Many people assume: “People who exercise must be healthy—why would they suffer sudden death?” This is exactly a misconception.

Director Li explained that many fitness enthusiasts hold the belief: “I work out regularly and stay in good shape. Minor chest tightness or palpitations are just tiredness; I can push through it.”

The truth is: exercise does not guarantee a troublefree heart. Some heart conditions are precisely exposed during exercise—such as silent coronary heart disease, hypertrophic cardiomyopathy, and inherited arrhythmias. These diseases show no symptoms at ordinary times, but easily trigger fatal arrhythmias and sudden death under increased physical exertion.

“Because you think you are ‘healthy,’ you are more likely to ignore warning signs,” Director Li warned. If you experience chest tightness, palpitations, dizziness, or shortness of breath during exercise, stop immediately and seek medical help—do not push through. Persisting could mean missing the final chance of survival.

Director Li further emphasized that young and middleaged people with longterm smoking, staying up late, and high mental stress, as well as patients with hypertension, diabetes, and hyperlipidemia, are all highrisk groups for sudden cardiac death. In addition to controlling “three highs,” quitting smoking, limiting alcohol, and maintaining regular routines, regular physical examinations can detect problems early for timely intervention. Electrocardiogram, echocardiography, and Holter monitor are basic tools for cardiac screening.
